I've been looking at several aftermarket exhaust systems.
bigexhaust.com has a 4" down pipe and 5" all the back
for $335. Is it worth upgrading to a system like this or should
I just get a aftermarket hiflow straight thru muffler and save
the my money for something else?

A turbo engine exhaust has the same design concerns for gas and diesel.

To minimize backpressure and maintain flow.

Going from say 2-1/2" to 5" across 6" of travel is'nt good for flow.

I would start with a aftermarket muffler and tailpipe you can retain should you decide to upgrade further.

So either a 4" straight thru or 5" straight thru single. Make sure the input of the muffler matches the output. So you just use a cone to go from 4'" to 5" between the stock pipe and muffler. That way you just add the 5" between the muff and dp at a later time.

From the turbo to the muffler is 3.5" (stock)
From the muffler out to the bumper is 4"(stock)

The system I was looking is
from the turbo the the muffler is 4"
from the muffler to the bumper is 5"

If I go with just a aftermarket muffler I'll need
a 3.5" inlet dia and a 4" outlet dia or
4" for both and get the adapter

Is it worth it?

I know a guy who has that exact system on his '03. The exhaust systems aren't going to gain you any power, but they might help a little with throttle response and EGT. The main benefit will be the sound ;D. His truck sounds good going down the road... much better than stock. If you plan on bombing later you're already that step ahead. It's worth the money for the big exhaust in my opinion.
