I just posted this same thing on another post::::
For what its worth, my 24 valve does this same thing, but only when its cold. And I have determined two things about my case. One: its the cold fuel that does it, not cold air or a cold engine, and TWO: more timing makes it much worse, since if I remove my EZ box, it gets better, but its still there. With the EZ on, in the winter, it can be wicked. Just like you, no one at all has been able to figure out why its doing this.. Its smooth as silk though in the warm weather....

if the shudder seems to be driveline related, and not engine related (by where it feels to be coming from) it is also possible that it is the center support bearing on the rear d/s. I was reading in my FSM that it can cause a shudder under high load. Normally, it is in the lower gears.
The fix is to lower the center bearing in 1/8" increments using flat plate or shim stock, not washers, until the shudder goes away.

My truck had a shudder when cold that felt engine related. It was not a vibration, it felt like a continuous surge - similar to very rapidly working accl. pedal. It would do it whether on cruise or not. Ironically, it also started leaked transmission fluid soon after. Turned out that the bolts that held the transfer case to the transmission came loose . I pulled them apart replaced the trans seal, and tightened the bolts up real tight - no more shudder.
