I got a pac-brake on my 96 dually 5spd and it works pretty well. It does seem like it takes a little long to come on and off but I don't know if that normal or not.

BD and PRXB, plus the Decelerator (pro version or whatever it is called) are the only proportional brakes, others are fixed orifice brakes and lose efficiency at low RPM. PRXB is popular and has good reputation but in comparing with BD, it has more complicated mechanical linkage to provide proportional exhaust throttling. BD relies on a very simple off-center butterfly for proportional force sensing and response. BD is also available in vacuum operated model (as well as air pump driven one). I chose BD for the simpler (read: potentially more reliable) design and simpler installation (I did not need an air compressor, I have one for tires).
If you'd like, you're welcome to give me a ring and try driving my truck to see what BD does. I am sure PRXB is very similar. The BD provides usable braking all the way down to 1200 rpm.
