they weren't designed to be run above 40psi without precision balancing

I don't think the shaft failed...I think the casting let go first, and the shaft had to let go. But at 150000+ RPM, it doesn't matter what went wrong, it's gonna grenade

The carnage reminds me of over spooling a green turbo.
Next thought is a massive turbo bark. Charge air tries to reverse comp wheel rotation especially when dropping from 40 psi. A larger comp wheel is going to catch more air.
