does it smoke off the line hard? If not your lever is prolly riding under your plate. also timing hurts your bottom end it helps mid range and top end, it's about finding a happy medium 16* is pretty goo

help
My bet is your governor lever is going under your plate. I am not sure of the #5 profle but it is probly defueling were it should be making peek torque. I know mine was I had a #8 plate and anything more then 1/2 forward the lever would go under then defuel in the mid rpm range.

It sounds like a timing issue. Also, move the starwheel back to the original position or you will fry your torque converter once you have the timing properly set.
One other thing to check is the little air line t at the back of the AFC housing. Be certain that the o-ring did not fall out of the line when you removed the AFC housing to do the fuel plate. Also, make sure your AFC housing is mounted all the way to the front of those little elongated holes. It is is to the back, your AFC will delay releasing and really cause a bog.
Piers has a good procedure on his website on how to set the rack follower lever if that is a problem, but I suspect it simply timing based on the smoke comments.

im certain one could be the governer arm adjustment, if your arm is going under your plate, then you can expect the truck to be slow in taking off, and then it will light all of a sudden, (happened to me). However, if your arm is correct, I wouldnt know if it is your timing, they are supposed to run harder on the top end when they are timed, perhaps yours did slip. I would also worry about how far forward that starwheel is, and you might wanna try bumpin ur plate forward to, just to see what kind of reaction you get when u floor it again, you might get more smoke, but just play with that starwheel and get it where u like it..
