Re:coil spacer?
I've never done a 2wd, but it should be fairly similiar to the 4wd. Take the shock off, loosen the sway bar, and the spring should come out with some help. Put the spacer on top of the spring, put it all back together. You shouldn't have to do any other modifications, unless you want to put some better shocks on while you've got the factory ones off. If not, the stock ones should work with the lift. It wouldn't hurt to get a front end alignment done.

I found http://www.jackit.com/dodge/dodge_leveling.htm these kits. I'm lookin at the deluxe kit with steel spacers and Bilstein shocks. Replace the OE crap shocks and steering stabalizer too.
I just finished e-mailing the sales department to find out if the kit will fit an '04 dually 4x4.
Levelled out, Bilstein shocks, and coupled with rear air bags, it should be a sweet and smooth ride over factory. 8) And, the factory ride from the '03's I test drove weren't terrible to begin with. Just the OE shocks don't last.

Jack-It knows their business.
Lucky enough to live 15 minutes away from their shop, had them do my 5 inch lift.
Wish I would have gone there first when I attempted the 2 inch leveling kit, well had a tire shop attempt it.
