Nv5600 clutch noise hard shift

Nv5600 clutch noise hard shift
05 4x4 nv5600. When you press down clutch pedal the throwout bearing (maybe pilot bearing?) is real loud. Clutch also has chatter from a dead stop. Doesn't seem to be slipping any other time. 110k miles. Lastly the gears shift harder than my other truck. Its hard in all gears and there's no grinding.
Is it time for a new clutch or just throwout bearing (or pilot bearing)?

IMO, the effort/cost involved in removing the transmission vastly outweigh the cost of the clutch components. And considering there's 110K miles, I would replace all of it. Clutch disk, pressure plate, throwout bearing, pilot bearing and resurface the flywheel if needed. While you're at it, flush the clutch hydraulic line too.
