superlift and fabtech for the off the shelf kits come in 6 and 4 varieties. Would I recommend them? Not unless your truck is a major street queen. Superlift has issues with sagging coils in the front. If you want a better option, call Don Thuren of Thuren Fabrication or Sage Carli at Carli Suspension. They can both help you with a better suspension system. BDS is also worth mentioning.
Cost varies with options on the custom stuff, but the off-the-shelf kits wil run around 1200 for parts only.

Well im sure you're going to get 30 people jump right in saying kore, carli, thuren. While they are very nice.....thats not what you are looking for by reading your post. If you want a 4.5-6 inch lift any of the manufacturers standard arm location lifts are relatively easy to install yourself if you have the tools and some mechanical inclination. As far price it all depends what you want. Realistically for a lift that size with your looking from between 800-1300 and shocks could add between 300-1500 depending on weather you want regular old hydraulic or you have to have the newest remote ressy dual shock mounts.
Just for reference even though its not "exactly" what yer looking for I just ordered a 4" superlift kit with the bilstein shock upgrade for 13 bills delivered. You can go much cheaper or much more expensive. Really it's up to what you can afford to spend and what ou want to do with your truck.
Hope this helps you out man,
Tom
