03 brake issues

I never heard the brake indicators but deffinatly heard the metal to metal grinding the other day, just seemed to start all of a sudden, well I found the source to be the rear brakes, I have 64K on the truck and almost no towing, just a daily driver, the funny thing was that the inside rear pads were worn to the metal, but the outer pads had just under half left in them, and the front pads looked to be about half gone, its a dually but should this be happening? I reassembled everything and used a little grease on the sliders just incase it was binding.

Actually front pads wear out faster becasue they have all of that weight on them. Usually the rear pads will last about twise as long as the front pads.

Not in my 25 years or so of dealing with 4 wheel disc systems. On drum yes.But not 4 wheel disc on autos.Smaller pad, softer compound.Time will tell on the D.C.trucks.

That statement is incorrect for disc brakes. Was somewhat true for drum brakes.
The inside pad wears faster on the rear for many reasons, mostly because of dirt buildup on the sliders and rotors. Early Crown Vic's would wear the inner pad out quickly until a shield system was devised to keep the dirt away.
