I'm not a tranny tech by any means but I will tell you my story.
I had similar issues with my 02 but I DID get the shudder in 3rd gear locked only, 4th gear wouldn't make a shudder but rater a 2-300 RPM increase.
Fast foreward, I took the truck to Iowa to have the trans rebuilt by Dave (Goerend) The internals were all in good shape, he replaced one band but said it could have been reused but $110 for parts I put it in. New triple disk converter, 48re input VB and a few other misc parts the trans was done.
Next I was ready to drive away but he (Dave) insisted we crack open the old converter to see if it was really bad, I couldn't make it shudder or slip on his test drive. Lets just say there was less than a papers thickness of material left on the disk. He was amazed I made it to IA and hadn't done more damage to the trans itself.
This is my story and if I were to give a SWAG I'd bet torque converter.

Update
Ok the noise filter is installed and I believe that problem is resolved!
The OD "slip" is still there as defined in my first post. Tuesday I will take it in and have the pan dropped, new filter, valve-body lossened, and replace all 16 quarts with Dextron 3. I will report if the slip goes away per DTT advice.

UPDATE on TC slipping
OK, changed the tranny fluid to dextron3 from ATF4 (per DTS) and it did not help the OD acceleration / slipping problem. Old fluid looked fine with min sludge on magnet.
I am now going to go ahead and change the TC to a Billet and upgrade the valves in the Valve Body next week. Will post the results!
Thanks for all the help and suggestions so far!

It would only happen going up an incline, not steep hill just uphill. After I put the Dextron3 in it it actually happen more often and even on level ground.
In order to make it slip I had to accelerate "firmly" from around 60mph to 70mph without it downshifting to 3rd. I then get a 100 to 200 rpm slip even with the lock switch on.

Wally world fluid is paying a royalty fee for using ATF+4 chemical package. I'm sure it is the same stuff, probablly better than some highly advertised fluids that supposedlly work but don't use as good chemicals as ATF+4. Again JMHO

I had a similar problem with a brand new trans me and a buddy built. Brand new BD triple disc converter, it would not slip unloaded but put a trailer on it and get in the hills it would slip like crazy. I tried all the fixes you can think of and could not beleive my new tc was bad. Well I gave up and got a precision tc that was billet but a single disc, paid $400 bucks for it and problem solved. No more slip. Also at $4 a quart with the tc slipping all that material is going to go through the trans so get a tc quick or it will trash the whole thing.
