My guess would be one of the following:

Tires
Wheels
Wheel Balance
Tie Rods
Ball Joints
Steering Box
Front Bushings
Bearings

Narrows it down a bit, eh?

Yeah probably need an alignment :-D one bad front shock can cause your truck to shake also. Most shops i've been to charge minimal for an alignment until they have to fix anything. I think the one i used to work at charges $75 to scan it and then their $90 an hour to fix it(usually takes 30-45minutes if nothing is broken).

My truck has had several severe cases of death wobble, all at different speeds/conditions. Last case was a tire coming apart internally mixed with a loose steering box and loose left front shackle. it sucks because it seems any thing in the front end can cause wobble if the conditions are right(or wrong as it seems)

Do the easiest/cheapest things first. Try rotating your tires from front to rear to eliminate a bad balance or bad tire. That's what my problem was.

Maybe 18 years of normal wear and tear on a truck engineered on 1960's technology when the maximum legal highway speed in most states was 70 could have something to do with this?

idk, my 85 leaning tower of power would do 90mph and 30mpg doing it. My 91 does a little more than that and holds the road smoother at speed than it does chattering away at idle.
