My Dad is Buying a First Gen
My dad is looking at buying a 91, W250, 4x4, 5 speed, regular cab. 39K (Yes only 39K) original miles, original owner. I believe he said $11K.
What kind of issues should he look for in the first gen trucks. I have owned an 02 and an 06 CTD, but not one of the originals. I tried checking prices in the classifieds and online, but it is impossible to find anything with even less than 100K miles to compare.
I have heard that there are several really simple mods that can be done to the first gen trucks. Is there a list of these somewhere?

all the common problems and fixes along with perfomance mods are in the "sticky" currently the second, but usually the first thread.
as far as the price if the thing is absolutely perfect then i believe it could bring that elsewhere. is it intercooled? one went on ebay like that a year or two ago, 92 w250 4x4 5-speed 52K i think and it was up at 16-17k i believe. personally i sold a 92 4x4 5-speed ext cab orig owner with 118k on it for $7500 last year, probably could have gotten more but sold it to a guy i know.

There are some problems with ULSD and the seals (older style material is my understanding) on the injection pump, with that low of mileage, I wouldn't worry about it. Me personally I am having my injection pump dripping a bit but when I get the time, I'll get a new gasket kit from Bosch.
As for additives, run Howes or power service. ATF isn't meant to be burned and should you get dipped, I think you'll have a hard time explaining why your fuel is pink...
