When 1/8 inch or 1/16 timing inch is said. is it talking about space from the pump to the cylinder head? or is how much its moved on the hash marks on the timing case?

1/8'' is the separation of the mark on the timing case and pump

truck runs a little rougher but like it did before the valve adjustment i hear its harder to start in the cold weather but who knows the difference is great.
