Does it feel like the front end or from the driveline?

Did you leave those centering nuts on the front wheels? I didn't and hit a big bump and they went out!!! It was a pretty hefty pothole though. Anyway, centered them again and left the 2 centering nuts on this time. Vibration was gone.

The centering procedure is for DRW trucks only.

Have you checked your yokes for wear or a loose nut or slip and stub on your driveshaft?

don't forget about U-joints...I've seen and had more mystery vibrations that were caused by U-joints than you can shake a stick at! You can rotate a driveshaft all day long while looking at the joints and the caps, but if the cap is cracked or broken on the back against the yoke, you'll never see it until it's too late. I found out the hard way, had the mystery vibratoin for 3 months and couldn't find it, Was on the highway and it started doing it...so I floored it up to 75 and all hell broke loose. Busted the tailshaft housing and the rear crank case cover. That was one expensive U-joint!!!!!!!!

When you had the wheels balanced what was the road force of each of the tires?

I wasn't there when the shop put on shocks& balanced wheels. checked with shop today said he only balanced front 2 wheels. I ask for all 6 to be done. SOOOOO, i can't answer your question (93/flatbed).

I don't think they are SUPPOSED to be left in, but I do since I've had a big enough bump knock it out. Granted I didn't torque to 300ft-lb like we're supposed to on these 5/8" lugs, I just know it was "wicked tight". My torque wrench only goes to 150ft-lb.

You may have to remove the driveshaft to give the u-joints a good inspection. Joints with slop aren't the only source of a vibration, you may have one that is seized up.
