Why did the gasket blow?

Why did the gasket blow?

I changed my headgasket at thanksgiving, due to the fact that the cooling system was being pressurized and blowing all the coolant out of the radiator. I havent really put many miles on it since and yesterday it started doing the exact same thing, only this time oil was in the radiator and water was in the oil pan. I was told to replace the oil cooler, which i did with a $300 oil cooler and gaskets from cummins. It still does the exact same thing so i pulled the head to find that the oil jackets had all blown in between them and the cooling jackets. The the first gasket blew the cooling jackets on the right side of the motor and this time it is on the left. My question is what is causing this? I had the head machined and put in new bolts. 35-40 lbs of boost max, and 15.5* timing.

A lot of guys will tell you that you have to run low load and heat cycle the engine a few times, then re torque. You will also hear these same guys tell you that it should be done 3 or 4 times and this is the only way to prevent blow out.

How many retorques did you do on the new gasket?
Did you check to make sure the block was flat?

No retourques, Did the first torque following cummins procedure and was told that the stock bolts do not need retorquing. Yes, Block was flat.

Its not that the bolts need retorquing.
The retorques are to seat the gasket better.

So how many lbs do you put on the retorque?
