Its the vibration that is created by lugging in 5th, if you shift early from 4th it will put a large load on the engine which I'm guessing causes a vibration. That said overall my truck has been real good to me, 4 years and only the injection has failed and 5th gear. I have towed in 5th since I fixed it and have had no issues.
