Harmonic balancer coming loose?

In the past couple of weeks I put the pin kit in for my fluidamper that I ran all last year. All new bolts torqued down to 95lbs as the manual says and it has came loose 2 times now. I even used red loctite and they still worked loose within a half hour of running. This is on a pulling truck that has seen 5000rpm so I don't need this coming apart. Any suggestions?

Out of balance, damaged or defective. Get rid of it.

Was the old one having issues , lose , broke ect ?
Call the manufacture .
Its possible that the crank is damaged , bolt holes opened up a little , have a good machine shop take a look .
