Im pretty sure they all have the factory spacer. I think some years were hub centric and some are lug centric. That would effect it.

hub centric is when the wheel is centered with the hub, the lug nuts on that system are flat where they contact the wheel ..... the other lug nuts are cone shaped where they contact the wheel, and that centers the wheel..... i hope that answers your question

so your saying the ways the adapter is sitting in the pic above is how it should go on the hub, with the larger diameter out? the reason i ask is how do you tighten down the lug nuts?

ok, got it now. the way i was looking at it, it seemed like the adapter was made so you could get an deep socket and impact gun in there. i will try again tomorrow. i just didnt want to install my new wheels studs and find i did it wrong.

The way you have it facing out in your pic is the side you will install the studs in. The flange on the other end bolts to the existing rotor.

The duallies from 94-99 had an intergal hub/rotor assembly. 00-02 used those spacers in the 2wd apps. Is that spacer from a 2wd or 4x4? Double check your fitment on the rotor before pounding the studs in, incase it won't sit flush.

ok, tried it again it should go on. how tight shoulld this thing be. Like should it just slide on with no effort or must i used some force and work the nuts around.

How clean is the surfaces and how far did it go on? It shouldn't be an interference fit, should be a slide fit, may take some persuasion, don't suck it on with the nuts. But those adapters were never used on trucks that had serviceable bearings like 94-99 trucks. They were on the ones that had unit bearings, so they won't have the grease cap and hub extension sticking out as far. You may be binding on those before you get it seated. I would get some blueing and see where you are making contact before beating it on with a hammer and sucking it tight with the nuts.
