If it's an auto it could be the TC slipping.

Does it feel like the tires are out of balance? Or is it only under acceleration? It shouldn't be a clutch problem. Any white smoke, or is it in the drive line? I was thinking maybe check your fuel pressure.

any speed

it does it empty at 65 with light fuel applied
with trailer it shakes at 45 if you apply a lot of fuel it goes away
or back off and coast it goes away

truck is a friends and has 330k miles and drips oil alot
i wonder if something got oil soaked ,like a mount or something
the rear u joint was changed and it did not fix the shaking

If the suspension and transmission are ruled out, I would suspect dirty fuel injectors. The 12 valve injectors really should have the pop-off pressure increased by 15% or so to compensate for the lighter diesel we get now days, that will keep them cleaner longer and increase fuel economy and power back to pre-goofy diesel days.

possibilities

Fuel injectors is a good guess and you can give it a good dose of PS diesel kleen to try and fix it. Another problem may be a bad pilot bearing; when mine went it usually shook when taking off though. cd
