I wouldn't worry too much about the motor unless there's something blatantly wrong with it (rivers of oil dripping down crank case etc).
I would THOROUGHLY check as many components on the truck as possible...pull all 4 wheels off, inspect brakes, axle seals, ALL ujoints, ball joints, tranny fluid, gaskets...depending where you are, I would run a magnet over the quarter panels, bottoms of the doors (underneath the actual door too), cab corners and above headlights to check for bondo/rust, tire wear for alignment issues...

Mine is a 24v and I bought it with 270k and now have 315k. I only drive it around 10k per year. It hasn't been abused at all and looks like a 150k mile truck.
I have had to replace a lot of stuff recently. You just never know when stuff will wear out. I would expect some parts to fail periodically. In the past year I've done starter, battery cables, steering gear and power steering lines, air conditioning components and just yesterday I was leaking coolant and found that my radiator is cracked at the plastic tank on the side. I expected to have to work on it some when I bought it. I would suggest that you expect the same. All the above parts were fine when I bought it.
I'm not saying not to get it. I love my truck, just sharing my experience.
